Activate in the "Plugins" network admin panel using the "Network Activate" link. Add shortcode [site_setup_wizard] on the page where you would like to display this wizard for allowing users to create new sites. You can change settings of the plugin from Network Admin -> Create Site -> Options screen and click Save Options.
